Using a pillow under the knees takes the strain off hips and the lower back area. With a pillow under the knees, the hips do not have to work very hard to support the weight of the legs. Also, the pelvic position is neutralized, which relaxes the lower back.

Without the pillowing between the knees, the hips can become strained and the low back arched beyond normal.
